An Idaho in-law says a friend got giardia from using her teeth to help tie a dry fly knot in the leader/hook.

Wow, that seems crazy. Giardia is a micro organism, and getting sick is dependent on the number / concentration you ingest. Having just a few critters in your gut doesn't usually lead to symptoms.

This thread is gross. Giardia is alive and well...
The platter was licked clean? Favorite log to take dump behind? Jesus people, apply some personal backcountry hygiene and properly bury your waste. There's a reason you do this.
Every person venturing into the backcountry should read Manning's "Backpacking One Step At A Time". It's no longer published, not sure why. Yes, it's a book. Millennials: it's one of those paper things like on Grandma's shelf.

How could she rule out a glass of water from a restaurant, filling a water bottle from a garden hose or a dozen other possible vectors?

Fly fisher person here - you should absolutely be careful putting stuff in your mouth that's been in the river - the danger is reel.

^ This is much more likely.
There was a study done years ago measuring Giardia cysts in alpine lakes in the Sierras in the summer. The take home was that you would need to drink many, many gallons of water to ingest enough cysts (which isn't many) to get Giardia.
