The only way I can see someone being in "critical" condition from bear spray is in the presence of some pretty sever reactive airway disease (asthma). That would warrant a "critical" transport (ALS or Advanced Life Support in Washington) to an ER but would also likely end with the patient being discharged home after a couple of hours and steroids/nebs, especially if treatment was started fairly quickly.
As this is a hiking site after all, I guess it's worth adding that if you had asthma and if flared up in a severe fashion after inhaling bear spray while in the backcountry and hours away from assistance.... That could end poorly.

I would think a severe Anaphylactic reaction to the ~ 10 ounces of capsicum released nearby indoors would plenty to require some hospitalization. That a lot of Pepper Spray.
I don’t hike much in the North Cascades. Carrying around bear spray for a very remote chance encounter with an aggressive black bear seems silly. An accidental discharge would be far more likely than one in actual need.

Never fry jalepenos.

I carry a can of bear spray but have left the security zip tie on. I figure the worst case scenario is if something sneaks up on me while I'm in the tent, and hopefully will have fair warning and can deploy it then.
Frankly, the good Lord won't take me before my time and if that's in the woods due to a bear so be it.

Yikes! I had the pleasure of having my bear spray drop out of my pack once. It hit a rock just perfectly so the safety popped off and it discharged a small puff, just as I was leaning down to pick it up while inhaling. Yikes! Even just a tiny amount had me coughing for awhile. It would be so miserable to have a mishap described above happen in an enclosed space.
