I usually use two cubes; this leaves enough room for plenty of liquid while at the same time cooling the drink at a respectable rate. However, on particularly hot days I often increase the amount of ice.
Too many variables to define. It all depends on the drink. A tumbler destined for a vodka-soda or white russian usually gets 4. A big cup at the mini-mart with soda gets filled almost to the top so the soda will be watery (I like it like that).
I have an ice crusher with me at all times. So my answer is one gazillion. I just got back from Oregon with a selection of unusual tequilas, some of them are quite good. Tequila Chamucos is perhaps my favorite, it is bursting with fresh agave flavor.
