There are lots of men who work with this, as their partner or wife or daughter may deal with this.
It seems as harsh and debilitating as a good Inguinal hernia with a guy.
Rest, ice (cool pillow in abdominal area) and ibuprofen.

No, 48,000 a day yes it's over the RDA but it is not harmful for a about six months and will get vitamin A levels back where they are needed to be.

I just got some of the Dear Kate brand period panties. They are expensive and the shipping is god awful slow, but they are a great back up (I too must wear the giant overnight pads.) They claim they hold up to 2 tampons worth, (get the "full" coverage) but I find just bleeding into my undies a little gross.
There are many different cheaper "period panties" on Amazon (apparently all ladies in Asia use them) but in my experience they make your butt sweat. The Dear Kate's don't make you sweaty.
There are other companies too, but you get a 20% off coupon the first time you order.
As for cleaning would she have room for a small garden sprayer, the pump up kind? Some warm water in that would make a pleasant shower (for others too?) We use a 2 gallon sprayer on our land for a shower.
As for taking iron/vitamin A, my doc kept trying to make me take iron, I tried all kinds and it has been a disaster. I finally felt so terrible I stopped, and lost 5 pounds in 2 days, I was so "backed up." I was still going to the bathroom, but apparently not enough. I actually don't know anyone who can successfully take iron.
I hope your friend can manage to have a great trip!

I have a brand of iron that I can take just fine. I've tried them all too. Pro easy Iron is hemi
iron, animal source and easy to digest. My serrum ferritin was down to 4 this April, that's an
emergency. Got it back up with Hemi Iron.

Not cheap but it works so it's worth it. But iron alone won't do it. You need the vitamin A too. You lose both each month and it sets up a vicious cycle.
aslo a can of smoked clams has 100% of your RDA for Iron.
